How do patients receive the intake form?

You send an invitation via email, SMS, or both. The patient receives a secure, time-limited link that opens the intake wizard in their browser — no app download required. It works on phones, tablets, and desktops.

What happens when a patient completes the form?

The completed wizard is submitted as encrypted, structured JSON and stored securely in your account. You receive a notification, and the submission appears in your dashboard. You can also receive the data via webhook for integration with your EHR or practice management system.

Do you provide a Business Associate Agreement?

Yes. A BAA is included with every Professional and Enterprise plan and is available upon request for Starter plans. We're happy to review your specific compliance requirements.

Can patients save their progress and return later?

Yes. Patients can save their progress at any point in the wizard and return to complete it later using the same secure link. Their data is encrypted and stored until submission.
